The short answer: Fullscript and MedGrid do different jobs. Fullscript is a supplement and lab-testing platform: you build a plan, your patient orders and it ships to their door, and you can also buy supplements wholesale for the clinic. Prescription-only products aren't sold there. MedGrid is where a verified practice buys its own stock across categories, sends 503A prescriptions to licensed pharmacies and keeps orders, invoices and staff in one account, with Genesis, MedGrid's built-in AI assistant, to help with the ordering side. Plenty of integrative and wellness practices could reasonably use both.
If you run a functional, integrative, longevity or wellness practice, Fullscript is probably already on your radar, and "Fullscript alternative" is a common search. The honest answer is that the two platforms overlap only a little. This guide sets out what each one does, using each company's own public pages, and says plainly where each fits.
Fullscript at a glance
Fullscript describes itself as a platform for supplements, labs, lifestyle and adherence. Here is what its own site and help center say:
- Cost: its pricing page says accounts are free for providers and their patients, with custom plans for health systems and enterprise.
- Patient dispensing: patients join through an invitation from their provider and order from the plan, with free shipping on patient orders over $50.
- Practitioner earnings: its help center explains that converting to a profit account lets you earn a margin on sales through your dispensary; new accounts start as no-profit accounts that give patients 10% off.
- Wholesale for the clinic: a wholesale channel for in-office dispensing, with free shipping on wholesale orders over $250 and a stated catalog of 350+ brands.
- Who can join: credentials must be approved before any wholesale or patient orders can ship, and only U.S. or Canadian licenses are accepted.
- Labs: Fullscript says you can order 2,500+ blood labs and specialty tests with a single login, and that all lab testing on Fullscript is cash-pay only. It acquired Rupa Health on October 10, 2024, which it says adds more than 25 lab partners.
- Quality program: every brand and contract manufacturer must verify active third-party cGMP certification annually, and Fullscript sends products from its inventory to independent labs for blind purity and potency testing.
- Prescription drugs: its help center says pharmaceutical (Rx-only) products aren't available on Fullscript due to regulatory restrictions.
- AI and automation: Fullscript Assist is an AI chat tool for providers built into Fullscript. Its help center says it can build draft care plans and search and compare supplements, that it is designed to support provider decision-making, not replace it, and that it doesn't place orders. Patients get refill reminders by default and can schedule auto refills.
- Integrations: standard EHR integrations are included in the free plan.
Fullscript also owns Emerson Ecologics: it announced it had completed that acquisition in March 2022, and Emerson's site now calls itself a Fullscript company. Wellevate, an earlier dispensary platform, closed on October 1, 2023, and its accounts moved to Fullscript.
MedGrid at a glance
MedGrid is the platform doctors use to run the buying side of their practice: wholesale ordering from verified vendors, prescriptions to licensed pharmacies, invoices and staff logins, with Genesis, MedGrid's built-in AI assistant, helping along the way (see what Genesis can and can't do).
- Genesis, for the ordering side: ask Genesis where an order is, whether it has shipped or what is in your cart, and it answers from your own account. It can tell you what an order is waiting on, list what you're due to reorder from your own order history, and prepare a reorder or a hand-off to checkout that you confirm. Genesis never places, pays for or transmits an order on its own.
- Verified on both sides: clinicians sign up free, and your NPI is checked against the federal NPPES registry as you sign up. Vendors are approved by MedGrid before they sell, and the licences and registrations they file are marked verified once MedGrid has checked them.
- Catalog: 2,000+ products and 200+ brands, and growing, from clinic supplies and devices to skincare, supplements, aesthetics and compounded medications.
- Prescriptions: 503A prescriptions go to pharmacies, and the prescriber chooses from pharmacies filtered by the patient's state, using the states each pharmacy lists. 503A prescriptions are the only products that ship to a patient; everything else ships to your office.
- Price and control: ask for a better price on any product, with a competitor's invoice if you have one, and an approved price applies to every order after. Staff get their own logins with an ordering switch you control.
MedGrid vs Fullscript: side by side
| Fullscript | MedGrid | |
|---|---|---|
| Main job | Supplement plans and dispensing to patients, lab testing, and wholesale supplements | Wholesale buying for the practice, 503A prescriptions to pharmacies, and order admin |
| Who places the order | Usually the patient, from your plan; the clinic for wholesale | The practice (or a pharmacy prescription written by the prescriber) |
| Where it ships | The patient's address, or the clinic for wholesale | Your office; only 503A prescriptions ship to a patient |
| Prescription drugs | Rx-only products not available | 503A prescriptions through licensed pharmacies; other prescription-labelled products bought as office stock under your licence, behind credential checks |
| Lab testing | 2,500+ blood labs and specialty tests, cash-pay | Not a lab-ordering network |
| Practitioner earnings on patient sales | Profit account option with a margin on dispensary sales | Not offered; MedGrid is for the practice's own purchasing |
| AI assistant | Fullscript Assist: drafts care plans and compares supplements; doesn't place orders | Genesis: orders, tracking, cart and reorders from your account; prepares actions you confirm; no clinical recommendations |
| Cost to join | Free for providers and patients | Free for clinicians |
Where Fullscript is the better fit
- Patient-direct supplement dispensing. If you want patients to order supplements from your plan and have them shipped home without holding that stock yourself, that is the core of Fullscript's model.
- Earning on patient purchases. Fullscript's profit account lets you earn a margin on dispensary sales. MedGrid doesn't offer a patient dispensary.
- Lab testing at scale. Fullscript's lab catalog and the Rupa Health tools are built for ordering and managing blood and specialty labs.
- Clinical plan drafting. Fullscript Assist is designed to draft care plans and surface evidence for provider review. Genesis deliberately doesn't give clinical recommendations.
- Refills and adherence. Refill reminders and patient auto refills are built in. Genesis doesn't set up recurring or scheduled orders.
Where MedGrid is the better fit
- Buying the clinic's own stock beyond supplements: wound care, gloves, syringes, devices, skincare and aesthetic supplies, all in one cart with one invoice trail.
- Prescription and compounded medications. 503A prescriptions go to pharmacies licensed for the patient's state, and office stock follows the rules for its class. See 503A or 503B? Ordering compounded medications the compliant way.
- Seeing who you buy from. Vendors publish their licences and registrations on their store pages, marked verified once MedGrid has checked them.
- Negotiating once. A price request, approved, applies to every order after.
- Running the ordering admin. Genesis answers order, tracking and reorder questions from your account, and your staff order under your controls.
Can you use both?
Yes, and many practices would. A common split is a patient supplement platform for take-home plans and labs, and a clinic buying platform for practice stock and prescriptions. Keep the two straight in your records: supplements you buy for the clinic are inventory you dispense or use under your state's rules, while supplements a patient orders from their plan are the patient's purchase. If you dispense in the office, check physician dispensing laws by state for anything that counts as a drug.
How to decide: questions to ask
- Who pays? If the patient should order and pay for supplements directly, you need a patient dispensary. If the practice buys and bills, you need a buying platform.
- What's on your invoices? Group 12 months of purchases by category. If most of it is supplements and labs, Fullscript covers more of it. If it's supplies, devices, aesthetics and prescriptions, MedGrid covers more.
- Do you prescribe compounded medications? Those go through a licensed pharmacy, not a supplement platform. Check the pharmacy's licence in the patient's state on the nonresident pharmacy map.
- What should the AI do? Clinical plan drafting and buying-side admin are different jobs. Decide which one you want help with, and check that any assistant asks before it acts.
- Who on your staff orders? Check how each platform handles staff logins and permissions.

Frequently asked questions
Is MedGrid a Fullscript alternative?
Only partly. Fullscript centers on supplement plans that patients order and on lab testing. MedGrid is where a practice buys its own stock and sends 503A prescriptions to pharmacies. If your need is patient-direct supplement dispensing, Fullscript is built for it; if it's practice purchasing, MedGrid is.
Does Fullscript sell prescription drugs?
No. Fullscript's help center says pharmaceutical (Rx-only) products aren't available on Fullscript due to regulatory restrictions.
Can patients order on MedGrid?
No. MedGrid accounts are for verified clinicians and their staff. A 503A prescription written by the prescriber can ship to the patient; every other product ships to the office.
Does Genesis place orders for me?
No. Genesis can prepare a reorder or a hand-off to checkout, and nothing happens until you confirm. Checkout and payment stay with you. It also doesn't give doses or treatment recommendations.
Is either one free to join?
Fullscript says accounts are free for providers and their patients. MedGrid is free for clinicians, and your NPI is checked against the NPPES registry when you sign up.
